Hospice Case Manager

As the Hospice Case Manager, you will be responsible for delivering high quality patient care. Hospice work is incredibly rewarding. The work you will do has a direct impact on the quality of life, at the end of life. You will work with a team of professionals to provide compassionate care to our patients. The Hospice Case Manager ensures that Hospice patients are eligible and appropriate for the hospice benefit! .

Apply specific criteria for admission and re-certif! ication to hospice. Re-evaluate patient needs based on patient status and condition.

Collaborate with other professionals including social workers, chaplains, home health aides, and physicians to ensure patient’s needs are met appropriately.

Initial and ongoing assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living.

Implement the individualized plan of care and recommend revisions to the plan as necessary.

Manage discomfort and provide symptom relief

Provide specialized nursing skills related to palliative and end-of-live care.

Qualifications and Experience:
Graduate of an accredited school of nursing. (Diploma or transcripts to be provided to agency)

Currently licensed in the Operating Stat! e as a Registered Nurse.

One year nursing experience recommended, experience in hospice care recommended.

A current health certificate, CPR card and TB test must be kept on file.

Must have a current drivers license, auto insurance and reliable automobile.

A criminal background check must be performed before hire.
