The Clinical Social Worker is part of the Army wide Behavioral Health support of Soldiers and families and provides services as part of a multidisciplinary health care team to patients who are eligible beneficiaries of the Military Health Care System as authorized by Army Regulation (AR) 40-3, Medical, Dental, and Veterinary Care. Clinical Social Workers short-term care models, patient and provider consultation models and education in conjunction with the DoD PDH-CPG to address behavioral health (BH) concerns and medical conditions related to deployment in service members and their Families. LCSWs shall provide BH screening, assessment, and direct care, specialty referral and longitudinal BH care management to post deployed service members and their Families with conditions and concerns related to their post deployment. These services are provided at no expense to the beneficiaries.

Services are provided in Primary Care Clinics, at Military Treatment Facilities (MTF) with Medical Holding Companies, and in non-clinical settings on military installations within the Continental United States (CONUS) andPuerto Rico. Behavioral Health Clinical Social Workers provide clinical assessments, treatment, case management, referral and education according to their professional discipline, Army Medical Department standards, and the Department of Defense (DoD)-directed Clinical Practice Guideline for Post-Deployment Health Evaluation and Management (PDH-CPG). Services are provided in accordance with the established principles, practices and ethics of both (1) the Clinician’s Profession (e.g., National Association of Social Workers (NASW), and (2) Department of the Army regulations. The Contractor and Clinician shall abide by MTF rules, regulations and bylaws including the Medical Staff Bylaws, as well as applicable Army regulations governing such areas as medical records, confidentiality, and reporting child/spouse abuse, etc. The Government will provide use of all available MTF facilities and support services, materials, publications and forms, equipment and supplies at each location.

QUALIFICATION REQUIREMENTS

Education and Experience:

• Masters or Doctorate Degree from a Council on Social Work Education (CSWE) Accredited School. Substitute or related Degrees or related experience in lieu of degree are unacceptable.
• Minimum of 2 years post-masters social work experience (within the last 3 years), preferably in delivering individual, couples or group clinical psychotherapy services.
• Valid, current unrestricted, and examination based Clinical Social Work license from aU.S.jurisdiction that allows for the independent practice of clinical services such as LCSWs, LICSWs, LISWs. Related experience will not be accepted in lieu of license requirements.
• Clinical Social Workers the duration of the contract.
• Current credentials must be maintained at the assigned MTF and require periodic updates.
• Computer skills to enable them to enter and retrieve data from the Composite Health Care System (CHCS)/AHLTA.
• Ability to enter data and prepare reports in MS Office programs including Microsoft Word, Excel, Power Point and Access.
• Knowledge and experience with military health system, people suffering from medically unexplained symptoms, Post-Traumatic Stress Disorder, and/or disasters are highly desired.
• Ability to communicate effectively orally and in writing.

Additional requirements include:

• U.S. Citizenship (certified copy of birth certificate or naturalization papers).
• Ability to work as part of a multidisciplinary health care team, work some evenings and weekends as needed.

• Ability to manage personal stress and must be adequately rested and fully physically and mentally capable of performing the duties required under this contract.
• Must meet all contract mandated occupational health requirements including immunizations and the PPD (TB) test as directed by the MTF.
• The LCSW atFortBuchanan,Puerto Ricoshall also possess the ability to read, write and speak the Spanish language equivalent to a 3/3/3 Defense Language Aptitude Test level.

REQUIRED TRAINING

• Continuing education required to maintain licensure. Time off to attend training selected by the individual cannot not be billed to the government.
• Training necessary to maintain current Cardio-Pulmonary Resuscitation certification (AHA or Red Cross only).
• Initial and recurring training required by local MTF commanders or their supervisors. These training and include program specific as well as site specific training such as CHCS training, safety training, utilization of the PDH-CPG, etc.
• In addition to the requirements to attend training defined above, the government may make available to Contract Clinical Social Workers ; government sponsored training opportunities such as conferences and professional development courses that may enhance their ability to provide services to eligible beneficiaries with the approval of the Contractor, the Contracting Officer, Program Manager

and the Clinical Social Worker’s supervisor. Attendance may be at government expense if funds are available.

WORK SCHEDULE & LOCATION(S)

• Required services may be scheduled between the hours of 6:00 a.m. to 7:00 p.m., Monday through Friday, excluding U.S. Federal Holidays. Workdays will generally consist of 8 hour days. A provider can be scheduled to work a minimum of 8 hours and a maximum of 12 hours per day. Work hours are typically 0730 â€" 1630.
• No overtime is paid. Clinical Social Workers will receive time off per clinic policy for hours worked in excess of 40 hours per week. No provider shall work more than 80 hours in a twoâ€'week pay period, except when authorized by the MTF commander and the COR. Overtime hours will result in a reduced schedule to compensate the provider on a basis consistent with Title 38 guidelines.
• Duty on weekends and outside normal clinic hours may be required based on mission requirements.
• Lunch periods are not payable periods, but will be scheduled by individual MTFs, clinics and offices for continuity of operations. The number and length of breaks will be determined by the local MTFs, Clinics and offices as appropriate. Commuting time to and from required place of performance is not compensable.
• The LCSW shall notify their assigned site supervisor when they cannot report to work due to illness or emergency. Notification as early as possible, but no later than two hours after start of scheduled shift is required
• Scheduled absences must be coordinated with the assigned site supervisor at least 30 calendar days in advance.
• Contract providers will be assigned to a primary duty station clinic. However, providers may be required to rotate to other clinics and Army facilities within a 30 minute drive of their regular duty station. Travel will be reimbursed according to the JTR.

ESSENTIAL JOB FUNCTIONS

The Clinical Social Workers shall function as behavioral health referral resources and subject matter experts in deployment-related health programs to the medical staff in primary care clinics and other medical settings. They shall develop and implement clinical programs that improve the care of beneficiaries with concerns related to terrorism, medically unexplained symptom syndromes and/or BH conditions related to their deployment. The Clinical Social Worker shall provide services to high-risk beneficiaries including but not limited to: Soldiers being evaluated for medical discharge from the service, soldiers WIA and their families, family members of soldiers KIA, and deployment related medically unexplainable symptoms.

The performance of the individual HCP under this contract is subject to day-to-day supervision, clinical oversight and control by healthcare facility personnel comparable to that exercised over military and civil service HCPs engaged in comparable work. Duties shall be performed under the general supervision of the following government supervisors; Medical Treatment Facility Chief, Social Work Services(C, SWS) or Chief of one of the other BH clinical services where the services are performed. Clinic Officers-in-Charge (OICs) may share in some supervision of Clinical Social Worker based on mutual agreement between the C, SWS and the Primary Care Clinic OIC. The Clinical Social Worker will be informed of their supervisory structure during the initial orientation period. Clinical Social Workers shall keep their Supervisor informed of in-process cases, case progress, and consult with the supervisor on major problems. Decisions and recommendations by Clinical Social Workers involving methods of treatment are subject to the same supervisory review and approval as those of GS employees performing the same services. All services provided by Clinical Social Workers shall be in accordance with applicableDoD,U.S.Army, installation and facility directives, guidelines, programs and administrative requirements.

Duties shall be performed under the general supervision of the following government supervisors; Case Manager (nurse), Supervisor or CBWTU Administrator where the services are performed. The LCSW will be informed of their supervisory structure during the initial orientation period. LCSWs shall keep their Supervisor informed of in-process cases, case progress, and consult with the supervisor on major problems.

• The LCSWs shall conduct psychosocial evaluations and provide therapeutic interventions including crisis counseling to individuals, groups and Families as needed to facilitate clinical case management and optimize care.
• The LCSWs shall assess and resolve complex social, economic and psychosocial problems that may impact on service members’ and their Families’ medical treatment while the service member is being evaluated for medical discharge from the service by providing

direct casework services, to include assessment, referral, consultation and education.

• The LCSWs shall evaluate the impact of diagnosis or lack of diagnosis with unexplained chronic symptoms on individual and Family systems as well as assess patient’s functioning within work, Family and routines of daily living and identify areas needing continued support and resources in order to assist patient in treatment.
• The LCSWs shall provide services to high-risk populations including service members pending medical discharge due to physical and or behavioral health illnesses and service members who were wounded as a result of their military duties.
• The LCSWs shall establish, implement, and maintain in partnership with clients, Case Managers, cadre, and community resources as applicable, and an optimum, comprehensive care plan for patients.
• The LCSWs shall coordinate behavioral health services for those clients who will reside in their catchment in order to ensure continuity of care. They shall facilitate transfer of the case to local resources as appropriate.
• The LCSWs shall help clients as needed, and provide aid with negotiating systems to acquire resources and services, including helping with paperwork completion, while consistently demonstrating a caring attitude and a professional relationship with staff members and TRICARE network providers and staff.
• The LCSWs shall provide Behavioral Health training to the Case Managers and cadre of the CBWTU to enhance their effectiveness in identifying behavioral health concerns and make appropriate referrals.
• The LCSWs shall educate clients, their Families, commanders and health care staff on community resources including information and referral for financial, housing, educational, employment and childcare resources as required by client’s needs assessment.
• The LCSWs shall maintain complete and accurate records and document all client contacts/services provided in accordance with professional standards as well asDoD,U.S.Army, installation and facility directed administrative requirements. They shall enter patient clinical and administrative data as well as utilization data and other data into automated systems and/or paper records and create reports from these data in accordance with local policies and procedures, Federal Regulations and directives from the Army Medical Department or its program administrators. All evaluations and other records of patients will remain property of, and subject to the exclusive control of, the U.S. Government. All reports and documents prepared by the contractor in fulfillment of this contract will become the property of the U.S. Government.
• The LCSWs shall be responsible for the completion of individual workload accountability in accordance with program requirements.
• The Clinical Social Worker shall participate, as appropriate, in departmental QA programs. Provide technical advice and professional opinions upon request from the MTF Commander, Clinic OIC, Chief, Social Work Services or other supervisors.
• The Clinical Social Worker shall attend and participate in patient care reports, patient care conferences, team conferences, professional staff conferences and any other appropriate professional activities only to the extent that such attendance and participation is relative to assigned cases and/or performance of contract services as determined by the MTF Commander, Chief Social Work Services, Clinic

OIC or other supervisor.

• The Clinical Social Worker shall present client cases in regularly scheduled case conferences under the direction of the C, SWS, clinic OIC, and or program directors.
• The Clinical Social Worker shall attend meetings, case conferences and training as required.
• Perform other related tasks and duties as assigned by the COR.
• The Clinical Social Workers shall not make use of outside specialty consultation without prior approval of the COR. Unavailability of ancillary assistance does not preclude the contractor from providing the scheduled/required services.
• Removal of Contract HCPs: At any time during the performance of this contract, the Contracting Officer, COR, or the department Chief may direct the contractor to immediately remove any contract HCPs whose actions or impaired state raises reasonable suspicion that clear and present danger of physical harm exists to a patient, other contract HCPs, government personnel or to the impaired individual. This provision will be used in emergency situations only and not for the purpose of bringing performance issues or other non-urgent concerns to the attention of the contractor. The COR or contract HCP’s supervisor may request contract employees believed to be impaired to submit to government provided drug/alcohol testing. If the employees refuse such testing, the Contracting Officer may request that the contractor replace the individual.
