Summary: Provides and directs Physical Therapy services to patients including assessment, treatment, program planning, implementation, documentation, and treatment team communication. Functions under physiciansâ orders and p ractices within scope of licensure and training at all times.
Essential Duties and Responsibilities include the following. Other duties may be assigned to meet business needs.
Evaluates patients to obtain data necessary for treatment planning and implementation. May conduct specialized evaluations as needed. Interprets and communicates evaluation findings to patients, family, and other staff as needed. Develops treatment plans, including short and long term goals, and methods to achieve identified goals. Goals may include reducing pain, restoring function, preventing loss of function. Establishes individual patient treatment schedules and coordinates with patient, family, and other team members. Implements and conducts treatment as outlined in treatment plan, adapting treatment to meet the individual needs of the patient. Provides treatment in accordance with plans and established standards of practice, department procedures, and productivity standards. Provides/recommends adaptive equipment as needed to meet patient goals. Trains patient, family, and appropriate staff in the use or application of the above. Educates patients on safety and environmental awareness issues Communicates patient status/needs with other treatment team members including Case Manager, Director of Nursing, Physicians, etc. Monitors patientsâ response to intervention and modified treatment as indicated to attain goals in accordance with physician orders. Develops appropriate home or community programming to maintain and enhance the performance of the patient in their own environment. Adheres to Company/department procedures regarding documentation of services Consistently demonstrates sound judgment in the evaluation, planning, implementation, and follow-up of patient therapy programs. Attends Company and departmental in-service meetings, patient coordination meetings, trainings, etc. Maintains safe and clean work area and adheres to company safety standards. Complies with Infection Control, Universal Precautions, and OSHA standards for healthcare professionals.
Job Requirements Education
- Position requires a Bachelors degree or higher in Physical Therapy from an accredited program
- Requires licensure within the state of Utah as a Physical Therapist or eligible for a Utah license
- Minimum of one to three years of experience in a rehabilitation environment preferred.
- Generally works business hours
- Requires good interpersonal communication skills
- Valid driverâs license with acceptable motor vehicle record
Reports directly to the Director of their primary department
Supervises and directs PTAâs or other staff in the implementation of the treatment plan
