Hours of operation is from 9am CST to 9pm CST, this position is slated for our late shift needs which is 12:30pm CST to 9pm CST.

Advanced computer skills are required for this position as well as 1-3 years of clinical experience.

POSITION SUMMARY
The Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote/influence members decisions related to achieving and maintaining optimal health status. Requires an RN with unrestricted active license.

Health Coach Consultants play a major role of Disease Management team whereby they are able reach out to members with identified chronic conditions and do assessments, enhancement of medical appropriateness and quality of care, monitor, evaluate and document care with ultimate goal! of impacting change in lifestyle for optimum health managemen! t.

Required Skills:
EDUCATION
The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.

LICENSES AND CERTIFICATIONS
Nursing/Registered Nurse (RN) is required

REQUIRED FUNCTIONAL EXPERIENCES
Functional - Nursing/Medical-Surgical Care/1-3 Years
