Description: St. Mark's Hospital is presently looking of RN - Acute Rehab, Full Time right now, this vacancy will be placed in Utah. Under the general direction of the Nurse Manager, independently provides individualized care using the nursing process for patients routinely seen on the unit. Demonstrates clinical competency! , nursing judgment, teaching skills, and a commitment to improving the quality of patient care. Independently performs general and unit-specific clinical skills. Contributes to the St. Mark's Hospital Department of Nursing and the nursing profession in a positive manner.

Key Accountabilities / Competencies

Department Proficiency:
1. Conducts a comprehensive initial nursing assessment for all patients admitted to the Rehabilitation Center - including completion of admission FIMs (Functional Independence Measures).

2. Systematically and continually assesses the health and status of the patient assuring the application of age specific standards. Evaluation of the patient's status and response to medical and nursing treatments/interventions is routinely documented.

3. Completes the Interdisciplinary Plan of Care within the initial four days of admission under the direction of the rehabilitation physician.

4. In collaborati! on with the patient/family, with regard to developmental, cult! ural and age specific differences, develops and implements nursing care plans to include problems, goals, evaluations and interventions.

5. Reports the patient's current medical status, functional abilities and potential barriers to discharge in the weekly interdisciplinary team conference.

Qualifications

POSITION REQUIREMENTS

Current licensure as a registered nurse in the state of Utah.

Maintains current BLS certification.

BSN preferred.

Two years experience in acute rehabilitation or sub-acute rehabilitation preferred but not required.

Successfully completes general nursing and unit orientation.

Effective oral and written communication skills.

Computer skills needed.

Ability to work with other team members to ensure quality patient care.

Strong problem-solving skills.

Good customer service skills.
