Exercise full professional discretion and technical judgment based on licensure in general duty settings, clinical nursing settings and/or in public health programs. . Incumbents are responsible to: â"Oversee and/or perform the following essential activities directly connected to ensuring the proper and efficient operation of the Medicaid home and communityâ"based waiver for individuals with severe physical disabilities: , â"Compile, coordinate and forward (to Medicaids Eligibility Services and BACBS respectively) necessary documentation to support timely medical and Medicaid eligibility determination for waiver applicants and recipients; â"Oversee and actively participate in the plan of! care process and periodically monitor the delivery of service! s; â"Assist recipients to identify local area waiver services providers, community based resources, natural supports and to make informed choices when multiple options are available to fulfill the individuals plan of care â"Assist recipients to obtain and maintain needed Medicaid (state plan and waiver) benefits; â"Instruct recipients in methods of identifying need and effectively communicating those needs to service providers; â" Instruct recipients in management of personal attendant(s) including interviewing, selecting, scheduling, termination, time sheeting, evaluating performance, back up coverage; â"Instruct recipients in addressing problems such as changing levels of personal needs, grievance procedures, emergency coverage, exploitation and abuse; â" Regularly evaluate the effectiveness of the waiver; â"Monitor, evaluate, train and when appropriate, delegate medical assistance services to personal attendants (only as authorized by the recipients physician and ! in accordance with state laws); â"Perform quarterly face to face visits, and ensure immediate recipient notification in the event of substantial changes in the recipients health, safety, local waiver program environment, or requests for changes in recipient services; â"Coordinate with the state Medicaid agency to recommend modifications to policies, procedures and standards; â"Design/provide/arrange as appropriate, the delivery of provider training and instruction; â"Conduct provider recruitment and oversight/Quality Assurance activities.
Identify and assist qualified individuals and agencies to enroll as providers of waiver services; â"Conduct outreach activities to identify and inform potential recipients, their families and interested others in the community about the waiver program; â"Ensure applicant/recipient rights, including rights to fair hearing; â"Coordinate waiver services with enrolled Medicaid Nursing Facilities; develop and implement procedure! s and protocols to facilitate clients transition between nursing facili! ties and communityâ"based settings; â"Develop and implement internal quality assurance protocols; participate in joint program reviews with the state Medicaid agency; â"Compile information, data and reports to support the above functions and as required by the state Medicaid agency and CMS.
