Description: Access Home Care and Hospice is currently interviewing Registered Nurse/On Call right now, this job will be placed in Utah. Job Summary: The On-Call Registered Nurse (RN) works closeley with the RN Case Manager and other members of the interdisciplinary team in order to provide skilled and professional nursin! g care for patients outside of regular business hours. The On-Call RN is responsible for fielding, prioritizing and responding to calls as well as making arrangemnts for necessary patient care during such periods of time. The RN develops implements and follows a plan of care that meets the specific needs of each patient and is in compliance with federal and state regulations, reimbursement guidelines and Access' policies and procedures. Requirements: Must hold a current RN license Home care, hospice or community health experience preferred Previous case management experience preferred Availabe to work on the weekends and after hours Ablility to promptly respond to patient needs within 30 minutes of receiving a call Reside within 30 minutes of an Access office Committed to delivering quality care Ability to communicate effectively in English Competency in general nursing theory, techniques and practice
