Nurse Practitioner

Location: Ogden, Utah

The Nurse Practitioner will conduct primary care patient visits in independent and assisted living facilities in an assigned geographic area of responsibility. They will conduct examinations independently with excellent clinical competence. The Nurse Practitioner wil l work collaboratively with the patient, family, and patient’s residential facility to develop individualized plans of care.

**This position will be in Davis/Weber County Utah**

Duties to include but not limited to:

• Conducts medical evaluations of predominantly elderly, frail, chronically ill home-bound patients in their home environment.
• Documents visit via EMR for medical record and billing purposes.
• Develops a treatment plan in collaboration with the patient and family and their goals of care.
• Evaluates patient medication and writes/orders prescriptions as needed.
• Orders and interprets diagnostic tests relevant to patient’s needs.
• Collaborates with multidisciplinary team members.
• Facilitates the patient’s transition within and between health care settings.
• Educates and counsels patients and their families regarding health care options.
• Facilitates access to appropriate supp ortive care services.
• Serves and protects the health care provider practice by adhering to professional standards, policies and procedures, federal, state, and local requirements.
• Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
• Seeing 8-10 patients per day.
• On-Call schedule (Phone triage afterhours) 1 week on call every 12 weeks.
