The LPN is required to possess the knowledge and skills to perform, report, and record assigned nursing care in the Primary Care Clinics, including, but not limited to:
- Assists patients with Oxygen therapy
- Administers medication to include IV medication if certified
- Focuses on clinic visits, exams and treatments with provider
- Uses motivational interviewing and health coaching to guide patients in order to promote patient self-management toward individualized life plan of care.
- Reviews daily schedules for patient care needs
- Assists with triage: messages from patients and face -to-face visits
- Processing Clinical reminders with appropriate consults and alerts as indicated
- Makes Pre-visit contacts
- Check expiration dates of meds and supplies
- Assists with orientation: PACT patient orientation and new staff.
- Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy. Monitors patient during the procedure.
- Adheres to standards of practice and safe technique in preparation and administration of medication.
- Demonstrates understanding of age- and population-specific characteristics that affect the health care needs of the Veteran.
- Promptly recognizes urgent or emergent patient care situations, seeks assistance and effectively intervenes as directed when appropriate.
- Based on the patient's goals provides patient and family health education, to include self-management, prevention and wellness.
- Interacts and communicates appropriately with patients and family members.
- Demonstrates professional behavior (i.e., respect, civility, ability to accept constructive criticism), effective communication and interpersonal skills.
- Adheres to principles of infection control, safety, and universal precautions.
- Adheres to all requirements in regard to reusable medical equipment.
- Responsible for minor adjustments of the PCP schedule prior to daily huddles (i.e., labs, appointments, hospital/ER discharges, staff shortage).
- Assists in the orientation and competency validation of students, peers and clerical staff.
- Serves as a resource and advocate in the protection of patient and family rights.
- Completes mandatory reviews, including motivational interviewing, in order to promote patient self-management toward individualized life plan of care.
- Supports patient-driven care in a safe, efficient and cost-effective manner, using evidence-based principles.
- Promotes patient and personnel safety.
- United States Citizenship. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- Education. Graduate of a school of Practical or Vocational Nursing, approved by the appropriate State Accrediting Agency and/or {accredited by} the National League for Nursing at the time the program was completed by the applicant.
- L icensure. Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
- English Language Proficiency. Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in both spoken and written English
GS-06: Completion of at least one (1) year of additional qualifying experience at the GS-5 level or equivalent, fully meeting all performance requirements for the GS-5 LPN/LVN.
GS-05: One (1) year of experience equivalent to the next lower grade level or equivalent
GS-04: Six months of qualifying experience as an LPN or LVN; or graduation from an approved school and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient c are facility that provided a practical knowledge of human body structure and sterile techniques and procedures; or graduation from an approved school of at least 24 months duration.
GS-03: None beyond the basic requirements.

The full performance level of this vacancy is GS-06. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-03 to GS-06.
Physical Requirements: This position involves sitting, standing, walking, stooping, bending, reaching, and lifting. It requires assisting patients with activities of daily living, ambulating patients, turning or repositioning patients, assisting with transferring patients to wheelchairs/stretchers, and transporting them to other departments or facilities. Mechanical devices are available and are required to be used for heavy or diff icult-to-lift patients. The work may involve pushing or lifting weight in excess of 50 pounds, going up and down flights of stairs and walking long distances. The position requires visual and auditory acuity at a level to provide safe and effective nursing care. The position requires the nurse to be able to perform Basic Life Support skills, skills required in the Prevention and Management of Disruptive Behavior to protect.
