The Case M! anager plans, organizes and directs home care services and is ! experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Initial and ongoing comprehensive assessments of the client’s needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.
Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
Perform nursing care according to the needs of the patient and as indicated in the written plan of care. Assist in the assessment of needs, formulation and review of patient care plans.
Have a good understand of Medicare guidelines.

Qualifications and Experience
Graduate of an accredited school of nursing. (Diploma or transcripts to be provided to agency)
Currently licensed in the Operating State as a Registered Nurse.
One (1) year nursing experience recommended, experience in home health care recommended.
Must have and maintain a reliable automobile.
A criminal background check must be performed before hire.
