Overview
Are you passionate about providing healing for those who are suffering?
FlexCare provides medical case management for nursing home eligible Medicaid clients. We h elp people move out of nursing homes and maintain independence in the community by identifying resources and coordinating services to meet their individual needs and preferences. RN and SSW teams then meet with the clients in the community and monitor them to make sure their needs are met. We coordinate with medical doctors, mental health and substance use providers, other service providers, families, Medicaid, and Medicare to help people have the highest possible quality of life.
Locations: Clients in Utah and Salt Lake County
Schedule: M â€" F, 8am-5pm
Pay: Commensurate with experience

Responsibilities
- Assessing potential enrollees for discharge from nursing homes to a less restrictive community placement while maintaining their health and safety
- Coordinating all facets of medical and psycho-social care
- Working closely with a multi-disciplinary team.
- Link and coordinate services based on the needs of the individual being served in order to maintain health and safety
- Providing RN Case Management services according to New Choices Waiver guidelines.
Qualifications
- Associates Degree or higher in nursing from an accredited university/college
- You have a Registered Nurse License current in the State of Utah
- You enjoy working with a primarily older adult population
- You are proficient in using MS Office, social media and you have an ability to learn new software program s quickly
- You are a highly organized individual in order to meet contracting requirements for paperwork and timelines for moving people out of nursing homes onto New Choices waiver
- You naturally have excellent communication skills when collaborating with team mates, community partners, nursing homes and assisted living facilities
- You are a thorough, tenacious problem solver.
- You thrive in an independent work environment.
- You enjoy spending time with people and interacting with them
- Professionalism and a standard of excellent customer service is second nature!
