The Clinical Pharmacist is instrumental in the care of patients, as well as the development of effective, professional relationships within the multidisciplinary team. Acting as a resource to other staff members, the Clinical Pharmacist is an esteemed and valued clinical provider of patient car! e.
The Clinical Pharmacist will provide inpatient and! ambulatory pharmaceutical care services in different areas of the hospital to promote the safe and efficacious use of drug therapy. Responsibilities include direct patient care, medication therapy consultations, protocol development, medication reconciliation, discharge counseling, and code participation. Serve as a member of the Float team by participating in group meetings, staff development programs, projects, and journal clubs. Participation in departmental and interdisciplinary committees to support the organization's efforts for performance improvement to improve patient safety, therapeutic outcomes, and cost savings is required. Serve as a medication therapy resource for other clinical pharmacists, pharmacy students, pharmacy residents, pharmacy interns, physicians, medical residents, nurses, and patients. This position will work collaboratively with other pharmacy staff and team coordinators and report to the Float team pharmacist lead.

Shift Details: T his is a PRN (as needed) position. This on call position is defined as working 0 - 40 hours a week with no guaranteed shifts. This position will require to work some weekends and holidays.
Department: This important position will support the inpatient pharmacy located in the Intermountain Medical Center.
Additional Details: Flexibility to work various shifts beneficial.
Minimum Requirements
Applicants must be eligible for pharmacy licensure in Utah and be able to dispense controlled substances.
Current knowledge of state and federal regulations and statutes regarding the practice of pharmacy and the distribution of approved drugs, controlled substances, and investigational agents.
Basic Life Support Certification (BLS) for healthcare providers.
Physical Requirements
Hearing/listening, manual dexterity, seeing.
Preferred Qualifications
Three or more years experience in an institutional pharmacy envir! onment.
Previous hospital experience or pharmacy residency is desirable.
