Registered Nurse - Intake (PRN)

Location: Salt Lake City Utah

Description: Intermountain Healthcare is in need of Registered Nurse - Intake (PRN) right now, this career will be placed in Utah. Performs telephone assessment of the needs of referred patients by determining appropriateness for admission to Home Health or Hospice and assist in coordinating services. Provi! des excellent customer service to all referral sources and participates in agency marketing effort by maintaining relationships with referral sources. Is responsible for triage and patient placement. .Acts as a clinical resource to the health care team. This process is accomplished by completing intake referral information including intake data, essential background information, insurance verification and plan of care.

This is a PRN, 8 hour a week, benefitted position. Shifts may vary. Can discuss further in interview.

Minimum Requirements

Current RN licensure in the State of Utah

One year acute patient care or Home Health and/or Hospice

Basic computer skills.

Excellent customer service skills.

Must be able to work independently, prioritize and perform detailed work in a fast paced team environment.

Ability to exercise courtesy, tact, and sensitivity with team members and customers.

Physical Re! quirements

Hearing/Listening, Manual Dexterity, Seein! g, Speaking

Preferred Qualifications

Bachelor's Degree in Nursing (BSN)

Homecare, insurance or other third party payer experience.

Prior leadership experience.

Horizon computer software experience.
